• Your referral letter
• Your Medicare Card and Private Insurance Details (if applicable)
• Details of current medications
• Any Xrays or results from relevant investigations (audiograms etc)
• Registration form (please print out and fill in prior to your appointment)

CANCELLING / RESCHEDULING APPOINTMENTS
If you need to cancel or reschedule an appointment, please do this as early as possible to ensure availability for another appointment or to allow the appointment time to given to another patient.

Underground paid parking is available beneath the RPAH medical centre (Secure Parking). Enter off Carillon Avenue. Metered street parking is also available.

Crows Nest Rooms
Underground paid parking is available in Alexander Street (2 hours free), Holtermann St (2 hours free), Hume Street (2 hours free), Nicholson Street (2 hours free) and Secure Parking (PAID - enter off Bruce Street). Unmetered and metered street parking is also available.
